Nurse consult and review of patient questionnaire and cognitive assessment.

Specialist GP consult clinical assessment, examination and plan. Referral for appropriate tests and allied health input.
Allied health assessment and management
Consultations with our specialist allied health team including two consultations with our dietitian, and two consultation with our exercise physiologist.
Consult 3
Specialist GP synthesis of all your tests, consults and plans.
Consult 4
Specialist nurse consult dedicated to introducing you to programs and supports recommended by the healthy ageing team.
Ongoing reviews with GP, nurse and allied health as required.
Our Fees
Our care program has been specifically designed to keep you at home for longer.
All of our health professionals are committed to supporting you and have done extra training to do so.

Our fees depend on the bespoke program we develop for you. As a guide individual appointment costs are shown below. We will call or email you to discuss our packages in more detail once you complete the Registration of Interest.
Nurse consult fees:
Up to 30 minutes $75 per consult
Up to 60 minutes $150 per consult
GP Specialist per consult:
Long consultation 40 min $400 + $50 per additional 5 minute increment Medicare rebate $118
20-25 minutes appointments $220 per consult + $50 per additional 5 minute increment. Medicare rebate: $80.10
Investigations:
Costs for investigations vary according to provider – cost estimates will be provided at your appointment.
Supplements:
We provide detailed information on how to obtain any nutritional supplements we recommend. These are typically inexpensive. For convenience we provide these supplements for purchase from the clinic.
