Chronic Disease Management

GP Chronic Condition Management Plans are designed for patients with a chronic condition (e.g., diabetes, arthritis, asthma, cancer). A GPCCM Plan identifies your health and care needs and establishes the services to be provided by your GP, as well as the actions you can take to manage your condition. Appointments for a GPCCM Plan includes 15 minutes with our experienced Nurse followed by 30 minutes with your GP.

At this age, our lives are often busy. We are often at the peak of our careers and balancing this with looking after our children and possibly aging parents. It goes without saying that our own health and well-being often get pushed aside and possibly even neglected as a result. It is also a time when health problems can start creeping in and go unnoticed. Even if you are healthy, this assessment provides an opportunity to review aspects of your health that may have been overlooked during episodic visits, and provides a valuable baseline measure for evaluating your health and needs as they change from year to year.
