Chronic disease management planning is important for maintaining and improving quality of life.

At iHealth Centre, we offer several bulk-billed, chronic disease management options including, GP Management Plans & Team Care Arrangements, and Annual Health Assessments for eligible patients. These management options will give you a good understanding of your health condition(s) and equip you with a range of preventative health strategies.

GP Management Plan – iHealth Plan

A bulk-billed health plan for people with a chronic medical condition.

Team Care Arrangement – iHealth Plan

A bulk-billed health plan for people with a chronic or terminal medical condition who require ongoing treatment from a multidisciplinary team.

Annual Health Assessment

Free for Medicare-eligible Aboriginal and Torres Strait Islander people.

45-49 year Health Assessment – iHealth Check

A bulk-billed one-off assessment for people aged 45-49 years.

Annual 75+ Health Assessment – iHealth Check

A bulk-billed annual assessment for people aged 75 years

GP Management Plan – iHealth Plan

GP Management Plans are designed for patients with a chronic condition (e.g., diabetes, arthritis, asthma, cancer). A GP Management Plan identifies your health and care needs and establishes the services to be provided by your GP, as well as the actions you can take to manage your condition. Appointments for a GP Management Plan include 30 minutes with our experienced Nurse followed by 15 minutes with your GP.

Team Care Arrangement – iHealth Plan

Team Care Arrangements are designed for patients with a chronic or terminal medical condition (e.g., diabetes, arthritis, asthma, cancer) who require care from at least two collaborating health care providers (e.g., dietitian, exercise physiologist, podiatrist). Completion of a Team Care Arrangement activates a number of Medicare subsidised sessions with the coordinating Allied Health Care providers. Appointments for a Team Care Arrangement are included in the GP Management Plan appointment, 30 minutes with our experienced Nurse followed by 15 minutes with your GP.

Annual Health Assessment

Annual Health Assessments are used to improve your health and quality of life by identifying any factors that influence your physical, psychological, and social functioning. Health assessments may include blood pressure and sugar level checks, height and weight checks, blood and urine tests, as well as discussion about your family medical history and health priorities and goals. Appointments are bulk-billed and include 45 minutes with a nurse followed by 15-30 minutes with your doctor.

45-49 year Health Assessment – iHealth Check

At this age, our lives are often busy. We are often at the peak of our careers and balancing this with looking after our children and possibly aging parents. It goes without saying that our own health and well-being often get pushed aside and possibly even neglected as a result. It is also a time when health problems can start creeping in and go unnoticed. Even if you are healthy, this assessment provides an opportunity to review aspects of your health that may have been overlooked during episodic visits, and provides a valuable baseline measure for evaluating your health and needs as they change from year to year.

Annual 75+ Health Assessment – iHealth Check

Annual Health Assessments for people aged 75 years and over are used to improve your health and quality of life by identifying any risks or hazards in your home, as well as factors that influence your physical, psychological, and social functioning. Even if you are healthy, Annual Health Assessments provide a valuable baseline measure for evaluating your health and needs as they change from year to year. Appointments for an Annual Health Assessment include 45 minutes with our experienced Nurse followed by 15-30 minutes with your GP.
